A medical marijuana patient license allows an individual with an approved application to legally buy, use, and grow medical marijuana and medical marijuana products in Oklahoma. This license will be in the form of an identification card that can be used to prove an individual is a license holder. This card will contain the individual’s name, photo, date of birth, city and county of residence, the type of license, the date the license expires, and the patient’s unique medical marijuana license number.

- Start your online telemed consultation with the physician. During your consult, the physician will review the information you submitted, ask questions about your medical history, gain an understanding of the ailments you suffer from and answer any questions you have about the medical marijuana program and Oklahoma
- After your evaluation has concluded and you’ve been verbally approved, you’ll need to submit payment. This is an automated process as you’ll need to put in your credit/debit card details before starting the evaluation. Your debit/credit card will NOT be charged unless you are approved
- You will be emailed to your provided address a completed and signed version of the Oklahoma Medical Marijuana Authority Physician Recommendation form which is needed to obtain your card.
- You will submit this form to the Oklahoma Medical Marijuana Authority portal which can be found here. If you’re a new patient, click the blue ‘APPLY NOW’ button and follow the registration instructions. If you’re renewing your card, click the orange ‘RETURNING APPLICANTS’ button and follow the registration instructions.
Please have proof of residency which can come in the form of a state ID, U.S. Passport or U.S. Photo ID, an Oklahoma ID card or a Tribal ID card.
Be prepared to discuss your medical history and why medical marijuana would be beneficial in treating your symptoms/improving your life. While not required having medical records, health summaries, notes from your PCP, therapist and/or chiropractor or prescription medications would be helpful.

Physician Evaluations & Recommendations are $139 for new patients & $99 for renewals. Your recommendation is valid for two years. Physician consultations are non-refundable if you are approved and accept the recommendation.
The state requires a $100 application fee for new patients and renewals. If you are able to provide acceptable proof of Medicaid (SoonerCare) or Medicare enrollment or hold a status as a 100% disabled veteran, the application fee is reduced down to only $20.
Application fees must be paid by Visa, MasterCard, or Discover credit or debit card. An additional credit card processing fee will be added to the transaction. These application and processing fees are non-refundable. The payment will not be refunded for applications not approved.
No. Medical marijuana is still technically ‘illegal’ at a federal level, therefore insurance companies must comply with federal rules and regulations, not state.
Applications and payment will be collected through an online system. This system is available at omma.ok.gov. You will need a valid email address to login to this system. This email address will be used to send notices once you create an account.

Once the application has been submitted, it will be reviewed by the OMMA. If it is deemed complete and meets all requirements, an approval letter with your identification card will be mailed within 14 days of your application submission date.
If an application is incomplete due to unclear or missing information or documents, you will receive email correspondence stating the reason(s) it is incomplete. You will be able to log back in to your account to make corrections. Your submission date is the date a complete application is provided.
If an application is reviewed and denied because all requirements have not been met, a denial letter stating the reason(s) for denial will be mailed within 14 days of submission.
If you have never received your card, and it has been 30 days since your application was approved, please log on to your OMMA portal and fill out a replacement card request. Please select “card never received” as your replacement request reason. Please ensure your address is correct prior to submitting a replacement request. If your address is not correct, please first submit an update application request.
Patients and caregivers who have received their card but need a replacement can now request replacement cards through the OMMA application portal. To request a replacement card, login to your account, select “create new application” and select “patient card replacement” or “caregiver card replacement.” The request will be answered within 14 business days. Please ensure your address is correct prior to submitting your request. If your address is not correct, please submit an update application request. Please check your email for replacement card application status.
A medical marijuana identification card and another form of identification should be presented any time you purchase medical marijuana at a dispensary or have marijuana taken to a processor to be concentrated. Law enforcement may also require your identification card to prove you are licensed to legally use and possess medical marijuana.
Caregiver licenses are available for caregivers of certain patients who can provide documentation from a physician that they need a caregiver. This documentation is part of the Adult Patient Physician Recommendation Form and can be provided by you when you submit your application or at a later date by accessing your online account. A patient can only have one licensed caregiver at a time, and the caregiver must submit an application meeting all requirements to receive a caregiver license. This includes the requirement that the caregiver must provide an Adult Patient Caregiver Designation Form signed by you
Yes, you may submit the Patient Withdrawal of Caregiver Form to withdraw a current caregiver. To designate another caregiver, a separate caregiver application with another Adult Patient Caregiver Designation Form signed by the patient will be required.

Yes. Qualified patients will be allowed to grow up to six mature plants in their private residence, in addition to six immature plants (seedlings).
Under SQ 788’s proposed regulations, licensed patients can:
- Grow up to six mature plants and six seedlings
- Carry up to three ounces of marijuana on their person
- Own up to one ounce of concentrates and 72 ounces of edibles
- Possess up to eight ounces of cannabis in their residence
Under SQ 788 & the emergency medical marijuana rules, patients who hold a medical marijuana card have the right to:
- Enroll in school without rejection based only on cardholder status
- Rent an apartment without eviction or rejection based solely on patient status
- Work at their place of employment without discrimination for having a card or testing positive for marijuana
- Receive healthcare without disqualification merely due to medical marijuana treatment
- Visit or have custody of their child without denial only based on patient status
- Hold a state-issued license without suspension on the sole basis of cardholder status
However, your patient card does not give you the right to:
- Use or possess marijuana at your place of employment without recourse
- Work, live or learn somewhere that could face loss of a monetary or licensing benefit under federal law due to your patient status
- Sell or distribute marijuana to others without a commercial medical cannabis card
